Grade Verification Form

(formerly Grade Report Form)

For homeschooled, eligible commuter, military dependent and private high school applicants only, the Grade Verification Form or Private High School Grade Verification Form must be completed by the applicant and the high school counselor or school official for 6th, 7th semester or 8th semester/final grades. The High School Counselor or School Administrator should be prepared to upload the current transcript when completing form.

The Promise applicant will initiate the grade verification process by clicking the button below for their appropriate applicant type. The applicant must provide the School Counselor or Administrator’s first and last name and school email address to complete his/her portion of the form. Once the student submits, a request will then be sent by email to the School Counselor/Administrator from wvcolleges via airSlate to complete their portion, verify grade information and upload the high school transcript(s). If dual enrollment courses were taken and have been completed, the college transcript may be uploaded or requested from a college as well; if the dual credit courses are not complete, the transcript must be provided at the conclusion of the college courses. If using parchment or another electronic format to send a transcript, send to Promise@wvhepc.edu.

You must remove the Social Security Number (SSN) from all documents before uploading to the form.

The Grade Verification Form is not the Promise application and can be submitted after the March 1 application deadline.

Private High School Grade Verification Form

(WV private high school applicants ONLY)

The Private High School Grade Verification Form is for those applying with a high school status of private high school graduate only and attending a school in West Virginia. Those attending a public high school or should not submit this form (unless specifically requested by our office), because grades are automatically sent to us from the WV Department of Education.

You must remove the Social Security Number (SSN) from all documents before uploading to the form.

The Grade Verification Form is only for those applying with a high school status of homeschool, eligible commuter, or military dependent only.  Those attending a public high school should not submit this form (unless specifically requested by our office), because grades are automatically sent to us from the WV Department of Education.
